linkages Ecosystem
We are currently building linkAges, a scalable network to support aging in place with a high quality of life.
linkAges is a disruptive innovation that complements traditional systems of healthcare with population-based solutions to wellbeing. As a community-based ecosystem it activates and engages the latent potential of individuals and communities to create interdependent interactions that support successful aging in place.
linkAges, will be built around the following key capabilities:
Personal Profile: A senior health profile that captures the health and social characteristics of an individual and can be used to create a personalized health and wellness plan including linking individual needs to other system components like resource matching, time banking for non-monetary service exchanges and signal detection programs.
Signal Detection: Human Sensing and physiologic mechanisms to provide continuous surveillance and professional support to manage chronic health conditions at home and enable anticipatory action to pre-empt the serious consequences of shifts in the health of seniors. Continuing work in Signal Detection will build upon our historic developments in this area including our work in creating the EMPOWER programs.
To support the development of Signal Detection as a component of the linkages network in April, 2012, the Innovation Center launched a Developer Challenge.
Time Banking: Creating micro-communities of people participating in a time banking network where service exchanges are measured by the hours offered or used and services offered are driven by personal interests.
linkages List: A Yelp-style resource referral system targeted specifically to provide information of interest to older adults and their caregivers.
linkAges: A Change Agent
As a change agent, linkAges seeks to leverage the inherent vitality and resourcefulness of older adults and all other members of diverse communities to activate collaborative systems to support aging in place. linkAges tightly connects and integrates individual talents and assets, as well as social services and caregiver support already in the community with partnerships with professional health care teams and tools. The highly functional coordinated linkAges ecosystem will empower all community members to contribute actively in creating environments that support aging in place and enhance quality of life for older adults.
