The Druker Center for Health Systems Innovation is committed to building visionary health systems that advance wellbeing from within the community. Our work will have a positive impact on critical quality of life outcomes for community members. It will also enhance the capacity of health systems to create comprehensive new models of community and population-based care that include a broader focus on social determinants of health.
Our current work on the linkAges platform engages and activates the potential of community-based networks to support successful aging in the community.
The Innovation Center was founded by the Palo Alto Medical Foundation. As a 501(c)(3) nonprofit organization PAMF is supported with the generous investment of philanthropic capital.
